PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.

This Waiver and Release of Liability ("Agreement") is executed by the undersigned participant, or if the participant is under 18 years of age, by the participant’s parent or legal guardian, in favor of Curran Soccer Academy LLC, its owners, officers, directors, employees, coaches, assistant coaches, independent contractors, volunteers, agents, representatives, sponsors, facility owners, lessors, and affiliates (collectively, the “Released Parties”).

1. Acknowledgment of Risk

I understand and acknowledge that participation in soccer training, athletic conditioning, camps, clinics, private sessions, group training, and related activities (the “Activities”) involves inherent risks, including but not limited to:

• Physical contact with other participants

• Slips, falls, collisions, and equipment-related injuries

• Muscle strains, sprains, fractures, concussions, or other serious injuries

• Illness or medical emergencies

I acknowledge that these risks may result from my own actions, the actions or inactions of others, the condition of the facility or equipment, or the negligence of the Released Parties. I voluntarily choose to participate in the Activities with full knowledge of these risks.

2. Assumption of Risk

I voluntarily and knowingly assume all risks, known and unknown, foreseeable and unforeseeable, associated with participation in the Activities, even if arising from the ordinary negligence of the Released Parties.

3. Release and Waiver of Liability

To the fullest extent permitted by Pennsylvania law, I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all claims, demands, causes of action, damages, losses, costs, or expenses arising out of or related to participation in the Activities, including claims based on ordinary negligence.

This release does not apply to gross negligence or willful misconduct to the extent such claims cannot be waived under applicable law.

4. Indemnification

I agree to indemnify and hold harmless the Released Parties from any claims, damages, liabilities, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to my participation, or the participation of the minor named below, in the Activities.

5. Medical Authorization

I certify that the participant is physically able to participate in the Activities. In the event of an injury or medical emergency, I authorize Curran Soccer Academy and its representatives to obtain emergency medical treatment deemed necessary. I understand that I am financially responsible for any medical care provided.

6. Minor Participant Acknowledgment (Required for Participants Under 18)

I am the parent or legal guardian of the minor participant listed below. I have read and understand this Agreement, have explained it to the minor, and consent to the minor’s participation in the Activities.

I agree to be bound by this Agreement on behalf of myself and the minor, and I accept full responsibility for the minor’s participation.

7. Media Release

I grant Curran Soccer Academy permission to use photographs, video recordings, and other media of the participant taken during Activities for marketing, promotional, educational, and informational purposes, including use on websites and social media platforms, without compensation.

8. Governing Law and Venue

This Agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Pennsylvania. Any legal action shall be brought exclusively in a court of competent jurisdiction located in Pennsylvania.

9. Severability

If any portion of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
